What does research say?

How do we read immune evidence — the field with humanity's greatest prevention triumph and its most famous overclaim?

Immune research spans the widest evidence range in this library: at the top, vaccination — the only intervention that trains immune memory, proven at species scale (smallpox: the only human disease ever eradicated); in the middle, the conditional supplement signals (D in the deficient, zinc with the right formulation, C in the extreme-exercise subset); and in the folklore basement, the megadose era's founding overclaim, still selling. The field's reading lessons are the estate's sharpest: population-conditional effects, formulation-dependent products, symptom margins versus disease prevention — and the tier distance between a vaccine that prevents a disease and a capsule that trims a cold by half a day.

How to Read Immune Evidence

The field that contains both vaccination and the megadose era teaches tier literacy better than any other:

  • The endpoint defines the tier: vaccines are graded on disease prevented — cases, hospitalizations, deaths; supplements are graded on symptom days trimmed. These are different currencies, and the aisle's core move is the exchange-rate fraud: a capsule that shortens a cold by eight percent marketed in the language of a vaccine that prevents a disease. The honest page prices each in its own currency.
  • Population-conditional is the field's signature: D's effect concentrates in the deficient; C's prevention lives only in extreme exercisers; zinc's protocol depends on the formulation and the timing. When the same molecule shows benefit in one population and null in another, the honest label carries the population — the aisle drops it.
  • The null is a result, and this field has the famous one: the C-prevention null in the general population survived fifty years of trials and Pauling's entire post-Nobel career. The category's persistence after the null is the marketing lesson, not the science lesson.
  • The training-ground data is observational but elegant: the Amish–Hutterite natural experiment (genetically similar populations, radically different asthma) and the farm-effect literature support the 'old friends' calibration story — honest about design, striking in effect, and supportive of the garrison-exercise frame (fiber, fermented, environment) rather than any bottle.
  • Time-tier evidence the shelf never touches: the sepsis hour-1 bundle, the anaphylaxis autoinjector, the vaccine schedule — the minutes-tier and memory-tier interventions that outrank every chapter of this page. The shelf has no role in the emergencies and no access to the memory; knowing what tier a claim belongs to is the field's entire literacy.

1798The Memory, Trained

Jenner's vaccination

Edward Jenner inoculates with cowpox to protect against smallpox — variolation's risks tamed into vaccination (from vacca, cow). The discovery that the immune system's memory can be deliberately trained, a century before anyone knew what the memory was made of.

1880s–1908The Two Armies

Metchnikoff and Ehrlich

Élie Metchnikoff discovers phagocytes (cellular immunity) after the thorn-in-starfish epiphany; Paul Ehrlich devises the antibody side-chain theory (humoral immunity). Rivals to the end — they shared the 1908 Nobel. Metchnikoff's later yogurt obsession founds the probiotics story our probiotics page carries.

1950s–60sTolerance & the Golden Age

Salk, Burnet, and the thymus

The polio vaccines (Salk's killed, Sabin's live) launch the vaccine golden age; Burnet and Medawar solve the self-tolerance riddle (Nobel 1960); Jacques Miller proves the thymus — the 'useless organ' — runs the T-cell school. The two armies and their discipline, mapped in one decade.

1975The Antibody Machine

Monoclonal antibodies

Köhler and Milstein's hybridoma technique produces unlimited identical antibodies — the tool that becomes diagnostics, the biologics era, and the research engine of modern immunology (Nobel 1984). The weapons factory, industrialized.

1980The Proof at Scale

Smallpox eradicated

The WHO declares smallpox extinct in the wild — the only human disease ever eradicated, achieved with a vaccine, ring-containment strategy, and global will. The immune system's memory, weaponized at species scale: the tier-one evidence this page's hierarchy stands on.

1990s–2010sThe Engineered Era

Biologics and checkpoint inhibitors

TNF inhibitors transform autoimmune disease; the checkpoint-inhibition discovery (Nobel 2018) releases the brakes on anti-tumor immunity — the immune system aimed at cancer; CAR-T cells engineer the patient's own lymphocytes. The regulation this page calls the system's masterpiece becomes medicine's most powerful lever.

2020sCurrent Era◆ Latest

The mRNA decade

mRNA platforms deliver pandemic vaccines at record speed — the memory trained by instruction manual instead of the antigen itself. The frontier: long-COVID immunology, tolerance-inducing autoimmune therapies, and the continuing public lesson that the greatest immune technology is worthless if the arm refuses the needle.

Martineau: Vitamin D and Acute Respiratory Infection

BMJ · 2017

POPULATION · Individual-participant data from 25 RCTs, ~11,300 participants, across 14 countries

FINDING · Vitamin D supplementation protected against acute respiratory infection overall (adjusted OR ~0.88) — with the effect substantial in the severely deficient (OR ~0.58 at <25 nmol/L) and with daily or weekly dosing outperforming large boluses.

LIMITATION · The overall effect is modest and population-conditional — the trial arms with deficient participants carry the signal; the bolus arms performed worst. The honest translation: test-guided daily correction for the at-risk, not blanket megadosing — exactly the framing this page and our vitamin-d3 page keep.

Meta

Hemilä/Cochrane: Vitamin C for the Common Cold

Cochrane · 2013

POPULATION · 29+ RCTs, ~11,300 participants — prophylactic and therapeutic vitamin C versus placebo

FINDING · Regular vitamin C did not prevent colds in the general population (RR ~0.97); duration was reduced ~8% in adults and ~14% in children; and prevention DID appear in extreme-exercise subsets — marathon runners, skiers, and subarctic soldiers halved their cold incidence.

LIMITATION · The tri-fold verdict resists simplification, which is exactly why the aisle simplifies it: null for general prevention, modest for duration, real for a specific population. Therapeutic (post-onset) C showed no consistent benefit. The subset is small but mechanistically coherent — the oxidative-burst demand of extreme exertion.

Meta

Hemilä: Zinc Lozenges and Cold Duration (Formulation-Aware)

Open Respir Med J · 2011

POPULATION · Adults across zinc-lozenge RCTs, analyzed by the zinc salt used

FINDING · Formulation-aware pooling: zinc acetate lozenges reduced cold duration by ~3 days, gluconate by ~1 day — with the formulation-dependence the naive summaries miss entirely (citrate, orotate, and tartrate release no ionic zinc and show nothing).

LIMITATION · The formulation-aware approach is the strength and the controversy (pooling selected salts); the effect is real but concentrated in the right-salt, right-dose, right-timing subset — the same grading our zinc page carries, unchanged on this shelf.

RCT

Lange: Sleep and the Vaccine Response

JAMA / Psychosom Med · 2003

POPULATION · Healthy adults vaccinated against hepatitis A, randomized to normal sleep versus restricted sleep in the nights after vaccination; antibody titers followed

FINDING · The sleep-restricted group mounted roughly HALF the antibody response at follow-up — the memory's construction measurably dependent on the nights after the antigen arrives.

LIMITATION · Small sample, single antigen — but replicated in the hepatitis B series and with partial-restriction designs; the mechanism (memory-cell trafficking and consolidation during sleep) is established sleep physiology. The cheapest immune intervention in the evidence base is a protected bedtime.

Obs

The Amish and the Hutterites: Farming and the Immune System

NEJM · 2016

POPULATION · Genetically similar Anabaptist populations — Amish (traditional single-family farming, barn exposure) versus Hutterites (industrialized communal agriculture) — compared for asthma and immune profiles

FINDING · Asthma prevalence ~5% in Amish children versus ~21% in Hutterite children, with fundamentally different immune-cell profiles — and in the study's mouse arm, Amish house dust protected the animals from asthma. The microbial-exposure training story in its cleanest natural experiment.

LIMITATION · Observational design — but the shared genetics, the divergence of farming style by historical accident, and the dust-transfer mouse data triangulate causality about as well as a human field study can. The 'old friends' calibration, not sterility, is the immune lesson — and no bottle replicates a barn.

RCT

Zakay-Rones: Elderberry Extract in Influenza

J Int Med Res · 2004

POPULATION · 60 patients with laboratory-confirmed influenza B, standardized elderberry syrup versus placebo, 5 days

FINDING · The elderberry group recovered ~4 days faster than placebo (symptom-score improvement within 2–3 days) — the small trial that built the berry's immune reputation.

LIMITATION · Small sample, single season, one preparation — and the subsequent record is mixed (a larger Australian trial null, a 2020 post-COVID arm stopped early). Products are rarely standardized to anthocyanin content, the cranberry-PAC lesson's weaker cousin: the evidence is small-trial hopeful, not category-established — Tier 3, sold at Tier 1 confidence.
