What does research say?

How do we separate proven findings from marketing hype?

Cardiovascular research is the largest, oldest, most rigorous evidence base in nutritional and pharmaceutical medicine — and its supplement chapter teaches the field's sharpest lessons: formulation decides outcomes (REDUCE-IT's purified EPA succeeded where STRENGTH's mixed formula failed), events outrank markers, and the strongest dietary intervention in history was a cuisine, not a capsule.

How to Read Cardiac Evidence

The field that built evidence-based medicine teaches its reading skills best:

  • Events are the currency: heart attacks, strokes, deaths — not lipid levels, flow-mediated dilation, or 'vascular health scores.' Markers support; events prove. Demand to know which a claim is quoting.
  • Formulation is the finding: REDUCE-IT (purified EPA, 4 g) positive; STRENGTH (mixed EPA/DHA, 4 g) null — same dose class, different molecule mix, opposite results. 'Omega-3 works' and 'omega-3 doesn't' were both wrong; which one, at what dose, in what form, is the literature.
  • Population defines the claim: REDUCE-IT enrolled high-risk statin-treated patients with elevated triglycerides — the result is theirs, not the general supplement-buying public's.
  • The AF signal is the honest asterisk: pooled high-dose omega-3 analyses show a small but real atrial-fibrillation risk increase — the reason 1 g+ dosing belongs in a physician conversation, especially for AF-risk patients.

Harvey describes circulation

William Harvey demonstrates that blood circulates — pumped by the heart in a closed loop — founding modern physiology in one slender book.

1912The Disease Named

Herrick describes coronary thrombosis

James Herrick establishes that the classic 'heart attack' is coronary artery occlusion — the disease finally has its mechanism.

1948Epidemiology

Framingham begins

The cohort study that invented the risk factor — hypertension, cholesterol, smoking identified as predictors, and prevention science born.

1967The Frontier

First human heart transplant

Christiaan Barnard transfers a beating heart into Louis Washkansky — proof the organ itself could be replaced; proof, too, of how much else matters.

1994The Statin Era

4S proves mortality reduction

The Scandinavian Simvastatin Survival Study shows statins don't just lower LDL — they save lives. Cardiology's pharmaceutical century reaches full power.

2019Current Era◆ Latest

REDUCE-IT & precision nutrients

Prescription EPA reduces events 25% — the supplement-class molecule proving itself at drug dosing — while SGLT2s and PCSK9s widen the prescription frontier.

RCT

REDUCE-IT: Icosapent Ethyl in High-Risk Patients

NEJM · 2019

POPULATION · 8,179 statin-treated adults with elevated triglycerides (135–499 mg/dL) and established or high cardiovascular risk

FINDING · Prescription purified EPA at 4 g/day reduced major cardiovascular events by 25% versus placebo.

LIMITATION · A prescription drug at a dose retail softgels don't match; hemorrhagic-stroke signal noted; the subsequent STRENGTH trial (mixed omega-3) was null — formulation questions remain open.

RCT

STRENGTH: Mixed Omega-3 in High-Risk Patients

JAMA · 2020

POPULATION · 13,078 statin-treated adults with high cardiovascular risk and elevated triglycerides

FINDING · No significant reduction in cardiovascular events with 4 g/day of a mixed EPA/DHA formulation versus placebo (corn oil).

LIMITATION · The comparator (mineral oil in the run-in) and the formulation differences from REDUCE-IT are debated — but the null stands as the boundary-drawing twin every honest omega-3 discussion must carry.

RCT

Q-SYMBIO: CoQ10 in Chronic Heart Failure

JACC Heart Failure · 2014

POPULATION · 420 patients with moderate-to-severe heart failure, 300 mg CoQ10 daily, 2 years

FINDING · Fewer major adverse cardiovascular events and lower cardiac death in the CoQ10 group versus placebo, as an adjunct to standard therapy.

LIMITATION · Modest sample with early termination; larger confirmatory trials awaited — an encouraging adjunct signal, honestly graded as pending confirmation.

RCT

Lyon Diet Heart Study: Mediterranean Diet After MI

Circulation · 1999

POPULATION · 605 heart-attack survivors, ~27 months (stopped early)

FINDING · A Mediterranean-style diet rich in alpha-linolenic acid reduced cardiac death and non-fatal MI by approximately 70% versus the prudent Western diet.

LIMITATION · Stopped early for efficacy (an ethical endpoint); single region and era; the effect's size has never been replicated by any single-nutrient trial — which is itself the finding.

Meta

CTT: Statins and Vascular Risk

Lancet · 2010s

POPULATION · ~170,000 participants across 26 statin RCTs

FINDING · Each 1 mmol/L LDL reduction lowers major vascular events by ~22% per year — with no threshold below which benefit stops.

LIMITATION · Trial-level pooling of statin RCTs; smaller absolute benefit in lower-risk populations — the benchmark every supplement on this page is measured against, and none approaches.

Meta

Omega-3 and Atrial Fibrillation Risk

Circulation / Eur Heart J · 2021

POPULATION · Pooled high-dose omega-3 RCTs (≥1 g/day)

FINDING · High-dose omega-3 supplementation associated with a small but statistically significant increase in atrial fibrillation risk — the honest asterisk on the REDUCE-IT era.

LIMITATION · Pooled trial-level data; absolute risk small and concentrated at higher doses — but it converts 'more omega-3 is fine' into a dose conversation with your physician.
