Chapter 02 · Medical Conditions

Common eye conditions.

What can go wrong with the camera — and which failure counts in minutes?

Eye disease splits by mechanism into four families: the vascular (diabetic retinopathy — the leading cause of blindness in working-age adults, and the reason our diabetes page's canvas is retinal), the pressure (glaucoma — silent for years, half of cases undiagnosed, stealing the visual field from the outside in), the degenerative (age-related macular degeneration — the center-out disease of the sensor's core, and cataract — the aging lens, treated by the most successful surgery in medicine), and the surface (dry eye — the tear film's failure, and the aisle's biggest market). And the field's unifying rule: sudden changes are emergencies measured in hours — the eye is end-organ tissue like brain, and 'wait and see if it clears' is the sentence that costs vision.

🚨
Safety First

🚨 Call 911 / emergency department NOW for: SUDDEN vision loss in either eye — painless or painful (possible stroke, retinal artery occlusion, optic nerve crisis); a curtain or shadow over your vision — possible retinal detachment (same-day surgery decides the outcome); a red, painful eye with halos around lights, nausea, or vomiting — acute angle-closure glaucoma; a new headache in anyone over 50 with jaw pain, scalp tenderness, or vision change — possible giant cell arteritis, where steroids must start BEFORE vision is lost; chemical splashes — flush 15–20 minutes with water first, then emergency care; and significant eye trauma. Same-day ophthalmic care for: sudden new floaters or flashes (retinal tear — treatable before it detaches), and any red painful light-sensitive eye in a contact lens wearer (keratitis — never sleep in lenses). Scheduled, non-negotiable: annual dilated exams with diabetes; glaucoma-suspect screening by 40; and the standing rule that blurred vision changing over days-to-weeks is an exam, not a pharmacy visit.

🩸

Diabetic Retinopathy

The glucose ledger: microaneurysms, leaks, and — in advanced disease — fragile new vessels that bleed and scar the sensor. The leading cause of working-age blindness; silent for years; prevented by glucose and blood-pressure control (DCCT's arithmetic, Chapter 5) and annual exams; treated in the anti-VEGF era better than ever. Our diabetes page's complication anchor.

🕳️

Glaucoma

The silent thief: drainage failing, pressure rising, the optic nerve's million fibers dying from the outside in — half of cases undiagnosed, vision lost never returning. Screened by pressure and nerve appearance (the 40-year-old baseline); treated with drops, laser, and surgery. The disease the annual exam exists to catch before you notice.

🎯

Macular Degeneration (AMD)

The center-out disease: the RPE gardener failing, the macula's cones starving — dry AMD (slow, common) or wet (blood-vessel invasion, the emergency version). The AREDS2 formula's 25% progression reduction is the supplement world's one disease-modifying RCT (Chapter 4's story); anti-VEGF injections transformed wet AMD from blindness-by-default to vision-maintained-in-most.

🌫️

Cataract

The aging lens: decades of accumulated layers clouding the optics — universal with enough time, accelerated by UV and smoking and diabetes. The fix is the most-performed and most successful surgery in medicine: a 10-minute lens replacement at 98%+ satisfaction. The disease where 'aging' and 'treatable' stop being opposites.

💧

Dry Eye

The surface failure: the tear film's three layers (lipid, water, mucin) under-supplied or over-evaporated — burning, grittiness, paradoxical watering. Screen-heavy modern life worsens it (blink rates halve at monitors). Managed with warmth, artificial tears, and the prescription tier; the aisle's biggest market and the DREAM null's territory (Chapter 5).

⛈️

Retinal Detachment

The emergency: the sensor peeling from its support — flashes, a floater shower, then the curtain. Higher risk after high myopia, trauma, and lattice degeneration; fixed by same-day surgery when caught early (a tear lasered is a detachment prevented). The reason sudden floaters are never 'just floaters' the first time.

Prevention Strategies

Eye prevention is exam discipline, light management, and systemic care — in that order of power:

  • The exam schedule is the highest-yield eye intervention: annual dilated exams with diabetes (non-negotiable — retinopathy is silent), glaucoma screening by 40 (earlier with family history or high pressure), and any sudden change as an emergency, not an appointment.
  • UV protection, lifelong: sunlight exposure accumulates into cataract and contributes to AMD — UV-blocking sunglasses outdoors are the eye's sunscreen (Nambour-grade habit logic, skin edition's neighbor). Never look at the sun or an eclipse — solar retinopathy is direct sensor burn.
  • Control the systemic diseases that own the eye's vessels: glucose (DCCT: tight control cut retinopathy onset ~76%), blood pressure, and lipids — the retina is end-organ territory exactly like the kidney and the heart (our hypertension and diabetes pages' engines, read from the eye).
  • Don't smoke: smoking roughly doubles AMD risk and accelerates cataract — the modifiable habit with the strongest eye-disease linkage in the data.
  • Feed the macula: the lutein/zeaxanthin arithmetic (Chapter 3) — greens with fat, eggs, and the pigment layer built from the plate.
  • The contact-lens rules, unhedged: never sleep in lenses, never rinse them in tap water, and remove them the moment a red painful eye begins — pseudomonal keratitis can ulcer a cornea in 48 hours, and 'toughing it out' is the story corneal specialists tell with their eyes closed.
