Chapter 06 · Scientific Evidence & Guidelines

What does research say?

How do we separate proven findings from marketing hype?

Bone research teaches the sharpest surrogate-endpoint lesson in nutrition: density scans and fracture outcomes are different currencies, regulators require fractures — and the field's most valuable recent findings are what NOT to do (megadose vitamin D boluses, calcium excess, rest-based 'protection'). We interpret the literature in the context of BHOF and Endocrine Society guidelines.

How to Read Bone Evidence

Bone trials reward knowing which endpoint you're being sold:

  • Fracture outcomes are the currency: hip and vertebral fractures are what treatment exists to prevent; bone-density (BMD) is a surrogate — supportive, predictive, but not the disease itself. Regulators require fracture data; marketing often doesn't have it.
  • Population defines the result: calcium + D cut hip fractures in institutionalized and deficient elderly — but show weak fracture effects in healthy community-dwelling adults. Both findings are true; only one gets quoted per advertisement.
  • Dose windows are real: the bolus-vitamin-D trials proved that 'more' can be actively harmful — 500,000 IU annually increased falls and fractures. Biology has optimal ranges, and bone has told us twice (calcium megadoses, D megadoses).
  • The null matters: vitamin D alone (without calcium) did not reduce fractures in community-dwelling adults in pooled analyses — a boundary-drawing result that separates D's real role (deficiency correction, falls) from the marketed one (a fracture drug).

1892Founding

Wolff's law

Julius Wolff formalizes what bone always knew: the skeleton adapts to the loads placed upon it — reinforcing where stress arrives, dismantling where it falls silent.

1941Mechanism

Albright's estrogen link

Fuller Albright identifies postmenopausal bone loss as an estrogen-deficiency state and treats the first patients with estrogen — the beginning of hormonal bone medicine.

1960s–70sThe Drug

Bisphosphonates arrive

Compounds first studied as industrial corrosion inhibitors turn out to be bone-seeking: they embed in mineral and disable osteoclasts. A plumbing chemistry becomes osteoporosis's first-line therapy.

1994Definition

The T-score era

WHO defines osteoporosis by DXA-measured bone density (T-score ≤ −2.5) — the disease becomes measurable before the fracture, and screening becomes possible.

2006Prevention Data

WHI Calcium + Vitamin D

In 36,282 postmenopausal women, combined supplementation produced a modest hip-fracture reduction, clearest in adherent women — the trial that anchors food-and-supplement guidance.

2010The Warning

Megadose D backfires

An annual 500,000 IU vitamin D bolus INCREASED falls and fractures versus placebo — the dose-window lesson that reshaped vitamin D guidance toward moderate daily dosing.

TodayCurrent Era◆ Latest

Anabolics & the treatment gap

Romosozumab and teriparatide rebuild bone rather than slow loss — while campaigns target the real bottleneck: the two-thirds of fragility-fracture patients who still leave the hospital untreated.

RCT

WHI Calcium + Vitamin D Trial

NEJM · 2006

POPULATION · 36,282 postmenopausal women

FINDING · Daily calcium (1,000 mg) + vitamin D (400 IU) produced a small hip-fracture reduction overall, statistically clearest in women who actually took the supplements.

LIMITATION · Community-dwelling, largely replete population; adherence dilution; 400 IU of D is below modern tested ranges.

RCT

Annual High-Dose Vitamin D (Sanders et al.)

JAMA · 2010

POPULATION · 2,256 community-dwelling women aged 70+, one annual 500,000 IU dose

FINDING · The megadose bolus group suffered MORE falls and MORE fractures than placebo — a dose-window violation, not a deficiency correction.

LIMITATION · Bolus protocol may not generalize to daily dosing; mechanism debated — but the result ended annual-megadose practice.

Meta

Vitamin D Alone and Fractures

Lancet · 2014

POPULATION · Community-dwelling adults across pooled RCTs

FINDING · Vitamin D without calcium did not reduce fractures in community-dwelling adults; benefit concentrates in deficient and institutionalized populations.

LIMITATION · Baseline vitamin D status often unmeasured; co-interventions vary; the null applies to the studied population and doses, not to deficiency correction.

RCT

LIFTMOR: Heavy Resistance Training in Osteoporosis

J Bone Miner Res · 2018

POPULATION · 101 postmenopausal women with low bone mass, 8 months

FINDING · Supervised heavy resistance training with impact loading improved lumbar spine bone density and functional performance; low-intensity controls declined. No training fractures.

LIMITATION · Small sample, single supervised facility, adherence-dependent — the safety profile belongs to supervised progression, not unsupervised gym improvisation.

Meta

Calcium + Vitamin D in Institutionalized Elderly

Cochrane · 2007

POPULATION · Frail and institutionalized older adults across trials

FINDING · Combined supplementation reduced hip fractures by roughly 20–25% — the classic positive pool, in the population where intake and synthesis are most commonly short.

LIMITATION · Population-specific: results in the frail elderly do not transfer to healthy community-dwellers, where pooled effects are weaker.

Meta

Bisphosphonates and Fracture Reduction

Lancet · 2008

POPULATION · Women with osteoporosis across pivotal RCTs

FINDING · Alendronate-class therapy reduced vertebral fractures by ~45–50% and hip fractures by ~40% versus placebo — the benchmark everything on this page is measured against.

LIMITATION · Rare atypical-femur and jaw-necrosis risks (very low absolute rates) shaped duration limits and drug holidays; long-term (decade-plus) data thinner than the pivotal trials.
