Vitamin B12🧬
Chapter 01 · Supplement Overview

Vitamin B12.

The vitamin your stomach escorts into your blood — and the one your brain cannot replace?

Vitamin B12 (cobalamin) is the largest and most complex vitamin — a cobalt-centered ring molecule made only by bacteria, arriving in the human diet exclusively through animal foods and fortification. It is required for two systems that cannot fail quietly: red blood cell production (its deficiency causes the megaloblastic anemia our anemia page maps) and myelin maintenance (the nerve insulation whose loss causes numbness, unsteadiness, and — untreated long enough — permanent damage). Its absorption is nutrition's most elaborate cascade — stomach acid, a dedicated transport protein, and a specific ileum receptor — and every failure point in that cascade (aging stomachs, gastric surgery, celiac, chronic acid suppression) creates a deficiency pathway. The modern framing: B12 is safe, cheap, and unreplaceable where it's needed — and the danger is never the molecule, always the delay.

🚨
Safety First — Read Before Supplementing

The timing rule, stated at full force: untreated B12 deficiency causes PERMANENT nerve damage. The neurological symptoms — numbness, tingling, burning feet, unsteadiness, memory change, mood change — are a medical evaluation, not a supplement trial. Caught early, the damage reverses; caught late, it stays. Second warning: high-dose folate can correct B12-deficiency anemia while nerve damage progresses underneath — the masking danger that makes 'diagnose first' the iron-clad rule. B12 itself is very safe; the molecule's safety profile is among the best in this library.

🧬
The Cobalt Ring

Cobalamin (B12)

A cobalt atom at the center of a corrin ring — a structure so complex that no plant or animal synthesizes it; only bacteria and archaea make B12. Animals concentrate it in tissue (which is why meat, fish, eggs, and dairy carry it), and the human body stores 2–5 mg in the liver — a 3–5 year buffer that both masks early deficiency and stretches the timeline of the vegan who stops supplementing.

Core Biological Actions
§ 1.1
Red blood cell maturation

B12 is a required cofactor for DNA synthesis in the marrow's rapidly dividing cells — without it, red cells grow large but fail to mature: megaloblastic anemia, our anemia page's second major cause.

Myelin maintenance

The nerve-insulation chemistry — B12's role in the methyl reactions that maintain the myelin sheath. Deficiency unmakes the insulation; the symptoms are the signal leaking. The brain page's comparison images show it.

Homocysteine clearance

With folate and B6, B12 clears homocysteine — the cardiovascular-association amino acid whose elevation tracks with B12 status.

Neurotransmitter support

B12 participates in SAMe-related methylation chemistry — the plausible link to mood that our depression page grades as association-strong, correction-modest.

The 3–5 year liver buffer

The body stores 2–5 mg — years of coverage. The buffer delays the vegan's deficiency and the post-surgery patient's alike: the timeline every clinician counts backward from.

No plant source

The nutrient's defining modern fact: unfortified plant foods carry essentially zero B12. The vegan supplement isn't optimization — it's the pattern's structural requirement.

Mechanism of Action — The Absorption Cascade
§ 1.2
1 Food B12 bound to protein; stomach acid + pepsin release it
2 Haptocorrin (R-protein) escorts B12 through the stomach
3 Pancreatic enzymes strip haptocorrin; intrinsic factor (from parietal cells) binds B12
4 IF-B12 complex absorbed at the ileum's cubilin receptors
5 ~1% passive absorption at high doses — the oral workaround

The cascade is the story: four steps, four failure points, and every one of them a clinical deficiency pathway. Aging stomachs make less acid (step 1 fails) and less intrinsic factor (step 3 fails — pernicious anemia). Gastric bypass removes the parietal-cell mass. Celiac and Crohn's damage the ileum (step 4). Chronic PPIs suppress the acid. Metformin impairs IF-receptor uptake (the mechanism debated, the association strong). And the workaround that makes 1,000 mcg oral pills work for nearly everyone: at high doses, ~1% absorbs passively, bypassing the entire cascade — the pharmacology our anemia page teaches as 'the dose is the mechanism.'

Cyanocobalamin — The Studied Standard

The form in the classic trials, the most stable on the shelf, and the cheapest per dose. The 'cyanide concern' is folklore: the cyanide moiety is a trace safely processed by the body's normal chemistry — billions of doses over decades confirm it. The body converts it to the active forms. The default, evidence-anchored choice.

Methylcobalamin & Hydroxocobalamin

The 'active' coenzyme form (methyl) and the natural food-storage form (hydroxo). Both work well; the superiority claims over cyanocobalamin outrun the head-to-head data. Dose and consistency beat the form debate — pick one and take it daily. Hydroxo's claim to longer action has some pharmacokinetic support.

The Only-Bacteria Vitamin

B12's origin story explains its dietary distribution: no plant or animal can synthesize it — the molecule is made by bacteria and archaea, concentrated up the food chain by animals that host the microbes or eat the animals that did. Herbivores get it from gut-bacterial production (in the case of ruminants, in specialized forestomachs; in others, via coprophagy — the unglamorous truth). Humans host B12-making bacteria too, but in the colon — downstream of the ileum where absorption happens. We make it; we cannot absorb it. The evolutionary irony that defines the vegan's requirement.

The modern food supply quietly solves and hides the problem: fortification. Nutritional yeast, fortified plant milks, fortified cereals — the same public-health engineering that restored population iron and vitamin D status delivers B12 to the plant-based eater who knows to read labels. The honest vegan plan is two lines: fortified foods daily OR a 25–100 mcg supplement daily (or 1,000 mcg 2–3× weekly) — and a periodic B12 check, because the 3–5-year liver buffer makes 'I felt fine for years' mean nothing.

  • RDA: 2.4 mcg (adults) — tiny, but only absorbable via the intact cascade or the high-dose passive route.
  • The vegan instruction: supplement or use fortified foods deliberately — every major dietetic organization's position, without hedging.
  • The at-risk omnivores: adults over 60 (up to 20–30% with reduced absorption), gastric-surgery patients, celiac and Crohn's patients, chronic PPI and metformin users — the anemia and acid-reflux pages' depletion links.
  • The test: serum B12 (with MMA/homocysteine where borderline) — the labs that convert 'maybe' into numbers.

What It Does NOT Do

The honest negative space — the aisle's B12 inflations:

  • It does not boost energy in B12-replete people. The B12-energy shot for the tired-but-replete is the category's most persistent placebo ritual — real benefit, real risk, zero evidence in the replete.
  • It does not need to be injected in most cases. The Stockholm oral-pernicious trials proved the 1,000 mcg oral route works even where the cascade fails — injections are for severity and preference, not necessity.
  • It does not reverse established nerve damage. Caught early, neurological B12 damage reverses; long-standing damage is often permanent — the timing rule that makes delay the danger, never the molecule.
  • It does not require the 'active' form. Methylcobalamin's marketing superiority claims outrun the head-to-head data; cyanocobalamin — the studied, stable, cheap standard — corrects deficiency every time it's actually tried.
  • It is not optional for plant-based eaters — the one claim in the aisle that is exactly, structurally true: no unfortified plant source, a 3–5 year buffer, and a supplement that costs pennies. The non-negotiable.
