What do the trials say — and why is the honest summary 'one landmark, mixed metas, a null, and an unresolved question'?
Chromium's evidence file is the metabolic supplement field's most instructive unresolved case: a positive landmark (1997, China) that never replicated, meta-analyses whose positive signals live in subgroups of small trials, a Cochrane null for type 2 diabetes, trivial pooled weight effects — and, beneath the trials, an essentiality question that regulatory bodies answer differently (the US sets an estimate; EFSA declined to set one at all). This chapter reads the whole file in order: the rats and the patient that founded it, the landmark and the nulls that define it, and the population-conditional reading that is the only honest way to hold it.
The field that runs on one unreplicated landmark teaches replication literacy better than any other:
Louis-Nicolas Vauquelin isolates a new metal from a vivid orange Siberian ore — its compounds span the color wheel, and he names it chromium, from chroma: color. Pigments, tanning, and stainless steel follow; biology waits two centuries.
Chromium-depleted rats lose their glucose tolerance; brewer's yeast restores it; the active factor is traced to chromium and named GTF. A trace mineral is assigned a metabolic job — and a research field is founded.
A woman on chromium-free intravenous feeding develops diabetic-like glucose intolerance and neuropathy; intravenous chromium reverses it. The first — and still strongest — human evidence of the mineral's necessity, produced only by artificial feeding.
Chromium chelated to picolinic acid multiplies absorption over the inorganic salts — the chemistry that made a supplement industry possible, patented and popularized in the era the category formed.
Chromium picolinate becomes a best-selling sports supplement on mechanism marketing; the controlled trials null the body-composition claims; the FTC charges the claim-makers (1996–97). The fortune is built and corrected in one decade.
180 patients with type 2 diabetes; 500 and 1000 mcg picolinate daily; dose-responsive HbA1c reductions — the field's positive anchor, standing unreplicated at comparable scale ever since.
Pooled analyses split by subgroup; the Cochrane review nulls type 2 diabetes; EFSA declines (2014) to set a chromium requirement; the essentiality reviews catalog the chromodulin model's gaps. The maybe mineral, honestly unresolved — and still selling.
POPULATION · 180 patients with type 2 diabetes in China, 500 mcg or 1000 mcg picolinate daily vs placebo, 4 months
FINDING · Dose-responsive HbA1c reductions (the 1000 mcg arm falling ~2 percentage points), fasting glucose improvements, and lipid benefits versus placebo — the field's landmark positive trial.
LIMITATION · Single trial, single region, never replicated at comparable scale in nearly three decades — the unreplicated-landmark problem that defines the category's evidence shape. The honest grade: promising, unconfirmed, and exactly as mixed as every subsequent pooling has found.
POPULATION · A single woman on long-term total parenteral nutrition, developing glucose intolerance, neuropathy, and weight loss refractory to all adjustments
FINDING · Intravenous chromium reversed the diabetic-like syndrome within weeks — the first human chromium deficiency, and still the strongest human evidence that the mineral is biologically necessary.
LIMITATION · A single case (with a small subsequent series in similar TPN patients) — the evidence class that founds hypotheses, not categories: every case required artificial feeding, and healthy eaters on normal food have never been shown to run short. The TPN cases argue essentiality; they say nothing about supplements.
POPULATION · Insulin-resistant, prediabetic adults, chromium picolinate vs placebo, metabolic-marker endpoints over months
FINDING · Improvements in beta-cell responsiveness and some metabolic markers in the insulin-resistant population — the population-conditional signal the small-trial file keeps pointing to.
LIMITATION · Small sample, surrogate endpoints, single research group — the early-tier grade this page assigns. If chromium helps anyone, these are the populations; the trials that would prove it at scale have not been run.
