Chapter 02 · Medical Conditions

Common thyroid conditions.

What can go wrong with the governor?

Thyroid disease affects roughly 12% of Americans in their lifetime — 5–8 times more women than men — and arrives in three patterns: the gland slowing (Hashimoto's, the autoimmune underactive disease), the gland racing (Graves', the autoimmune overactive disease), and the gland remodeling (nodules and goiter, the structural changes). Most are common, treatable, and — the thyroid's signature feature — silent for years while the warehouse drains or the dial creeps. 🚨 The emergency rules below run in both directions; the dial fails both ways.

🚨
Safety First

🚨 Call 911 for: myxedema crisis — profound coldness, confusion, extreme drowsiness or unresponsiveness with known or suspected hypothyroidism; and thyroid storm — high fever, racing irregular heart, agitation or delirium with known or suspected hyperthyroidism. Seek prompt evaluation for: a new neck lump, hoarseness, or difficulty swallowing (thyroid-cancer workup territory); and Graves' eye symptoms — pain, double vision, or vision loss — urgent ophthalmology to protect sight.

🦋

Hashimoto's Thyroiditis

The autoimmune underactive disease — the leading cause of hypothyroidism in iodine-sufficient countries. The immune system marks thyroid proteins as enemy; the warehouse drains over years. Levothyroxine is the treatment. Our thyroid-disorders page teaches the full spectrum.

⚡

Graves' Disease

The autoimmune overactive disease — antibodies locking the TSH receptor in the ON position. Racing metabolism, heat, weight loss, trembling. Antithyroid drugs, radioactive iodine, or surgery; the eye disease (orbitopathy) is its distinctive complication.

📦

Nodules & Goiter

The structural changes — lumps (nodules) and enlargement (goiter), increasingly common with age. The overwhelming majority benign; the workup (ultrasound, biopsy when indicated) sorts the few that aren't. Iodine deficiency remains the global goiter driver.

🩻

Thyroid Cancer

The usually-treatable cancer — papillary type dominates, and small low-risk tumors can now often be WATCHED rather than removed (the active-surveillance revolution, Chapter 5). The Chernobyl-linked type taught the organ's radiation lesson — Chapter 1's case.

Subclinical Thyroid Disease

The TSH drift — hormone levels still normal, the thermostat compensating. The gray zone where treatment decisions are individual: age, pregnancy plans, symptoms, and numbers all enter. Our thyroid-disorders page teaches the TRUST trial's honest boundary.

Thyroid Eye Disease

Graves' orbitopathy — the immune attack extending behind the eyes: bulging, double vision, rarely sight-threatening. Amplified powerfully by smoking (the strongest modifiable factor — cessation is eye treatment, not general advice).

Prevention Strategies

Thyroid prevention is narrower than most organ prevention — but the honest levers are real:

  • Iodized salt, not sea salt: steady small iodine intake covers the gland's raw-material need without ever approaching the window's excess edge — the population-level lesson of Chapter 3.
  • Never self-dose iodine or kelp: the window has two edges, and supplement-dose iodine finds both — the thyroid-disorders page's Chapter 7 teardown, reinforced at the organ.
  • If you smoke and have Graves': quit — smoking is the strongest modifiable amplifier of thyroid eye disease; one of endocrinology's cleanest causal levers.
  • Know your TSH: the single blood test that reads the whole thermostat — every adult with fatigue, weight change, temperature intolerance, or menstrual changes should know their number.
  • Timing discipline on levothyroxine: for the treated: empty stomach, 30–60 minutes before food and coffee, 4 hours from iron and calcium — the absorption rules are part of the prescription.
  • Neck awareness: a persistent lump, hoarseness, or swallowing change earns an evaluation — the nodule workup is quick, and the cancer it occasionally finds is usually treatable.
