Chapter 02 · Medical Conditions

Common skin conditions.

What can go wrong with the barrier?

Skin disease is among the most common reasons people seek care — eczema and acne alone drive tens of millions of clinical visits, and skin cancer is the most common cancer of all. The categories organize cleanly by mechanism: barrier failures (eczema), immune accelerations (psoriasis — 28 days of turnover compressed into 3–7), the follicle's oil engine (acne), the cumulative UV ledger (photoaging and the three cancers), and the healing failures that read internal disease (diabetic and venous ulcers). And the field's unifying rule: the skin shows the inside — diabetes, thyroid disease, liver disease, and nutritional deficiency all write on the surface, often before they announce anywhere else.

🚨
Safety First

🚨 Call 911 / emergency department for: skin pain out of proportion to appearance with rapidly spreading redness, swelling, or gray/blue discoloration — possible necrotizing fasciitis, where hours matter. Seek urgent care for: rapidly spreading redness with fever — cellulitis, needing antibiotics today; a rash with fever, blistering, or involvement of the lips, mouth, or eyes, especially after a new medication — possible Stevens-Johnson syndrome, an emergency; and widespread redness with sheets of peeling skin — a hospital-level pattern. Any changing mole (ABCDE: Asymmetry, Border irregularity, Color variation, Diameter over 6 mm, Evolution) warrants dermatologic evaluation within weeks — early melanoma is highly curable; late melanoma is not.

🌡️

Atopic Dermatitis (Eczema)

The barrier failure: filaggrin genetics + immune overdrive → the itch-scratch spiral of cracked, inflamed skin. Management is moisturizer-first (the clinical instruction), prescription-tier second — topical steroids, calcineurin inhibitors, and now biologics for severe disease.

⚡

Psoriasis

The immune acceleration: turnover compressed from 28 days to 3–7 — cells arriving at the surface before they're finished, stacking into scaly plaques. NOT contagious. Carries a comorbidity cluster (psoriatic arthritis, cardio-metabolic risk — the inside-out disease) and has been revolutionized by biologic therapy.

🔥

Acne

The pilosebaceous engine: oil + dead cells + C. acnes + inflammation, in the teenage near-universal and a persistent adult minority. Moderate-to-severe disease is prescription territory — retinoids, and isotretinoin for the severe end. The aisle borrows from a tier it isn't.

Photoaging & Skin Cancer

The cumulative UV ledger: wrinkles and spots on the cosmetic end; basal cell (the most common cancer in humans), squamous cell, and melanoma on the serious end. Prevention is SPF — the one intervention with an RCT (Chapter 5's Nambour story). Tanning beds are classified Group 1 carcinogens.

🍷

Rosacea

The flushing disorder: centrofacial redness with triggers (heat, alcohol, spice, sun), papules, and — importantly — an ocular form that deserves treatment. Prescription anti-inflammatory and antibiotic tiers; trigger mapping is the patient's side of the work.

🩹

Chronic Wounds

The healing failures that read the vasculature: diabetic foot ulcers and venous stasis ulcers — the healing cascade stalled by glucose, pressure, and failing veins. Prevention is diabetes care (our diabetes page) and compression; treatment is wound-clinic territory.

Prevention Strategies

Skin prevention has one giant lever, several honest habits, and a diagnostic skill:

  • Sun protection is the single highest-yield skin intervention in existence — the only one with a randomized cancer-prevention trial behind it (Chapter 5 tells the Nambour story). Daily SPF 30+, rain or shine; hats and shade for the long exposures.
  • The barrier basics are clinical instructions, not marketing: lukewarm water, gentle cleansers, moisturizer applied to damp skin within minutes of bathing — the wall's mortar maintained by habit.
  • Never tanning beds; no smoking. Both are proven, dose-dependent skin-aging and cancer drivers — the twin studies made the smoking ledger visible (Chapter 5), and tanning beds are Group 1 carcinogens with no safe version.
  • The monthly mole check (ABCDE): asymmetry, border, color, diameter, evolving — plus annual dermatology for high-risk profiles (fair skin, family melanoma, tanning history, immunosuppression).
  • Manage the inside: glucose control is ulcer prevention; thyroid and liver disease often announce on skin first; nutritional deficiency still writes on the surface in subtle modern forms.
  • The shelf honestly sized: every supplement on this page sits far behind SPF, the moisturizer, and the prescription tier — the grading Chapter 5 delivers.
